Do not include a condition blindly. Lead-based paint and post-2018 mold or pest violations can require additional affidavits or documentation, and a Defect status may mean the physical reinspection has already occurred.

- Confirm the applicant information, building registration, current form, fee, and submission instructions.
- Inspect every apartment and common area in the request before HPD arrives and plan lawful access.
Submit, prepare the building, and follow the result
Use the current application, pay the listed fee, organize access, and make sure the building is ready for an inspector who may observe additional conditions.
- List each eligible violation and confirm the cited location is corrected and accessible.
- Attach the documents and affidavits required by the order and complete the current dismissal application.
